Description: This program aims to achieve two critical objectives: (1) Reduce unnecessary visits to the Emergency Department (ED) for non-traumatic dental-related issues, and (2) Educate and assist members immediately following their ED visit by providing contact information for their assigned dental provider and facilitating timely scheduling to prevent future unnecessary ED visits for non-traumatic dental related issues.
The presentation will share how our partnership with PointClickCare provides daily reporting of all members seen in the ED, the phone number they provided at check in, diagnosis and chief complaint, and the process used to contact these members within 14 days. It will also include an analysis of aggregate data to identify the dental concerns prompting ED visits, barriers to receiving care prior to the ED visit (such as transportation, work schedules, and language barriers), and whether members attempted to contact their assigned provider before visiting the ED.
Additionally, the outcomes for these members will be reviewed to determine if this intervention has successfully prevented future ED visits for non-traumatic dental related issues. The ultimate goal is to ensure members seek all necessary care from their assigned dental home, thereby facilitating timely follow-up appointments and educating members about a 24-hour triage line available to assist them before resorting to the ED unnecessarily.
