Abstract: Objective: To develop and pilot an initial measure set reflective of a Patient-Centered Dental Home (PCDH) with lessons learned shared through a Learning Collaborative.
Methods: The measure set was developed through a consensus-based process with a PCDH steering committee. Three organizations with extensive experience in quality measurement piloted the measure set. The pilot sites included an FQHC, a local health department dental clinic, and a large health system. They implemented a PCDH measure set, comprised of eight child-focused quality measures identified as fitting within the PCDH care model, using electronic health record data. The quality measures were developed by the Dental Quality Alliance. The pilot organizations, along with the PCDH steering committee and others, participated in a monthly Learning Collaborative to share experiences and lessons learned about measure implementation.
Results: Measures focused on access (follow-up after emergency department visits), prevention (topical fluoride, sealants, caries risk documentation), care continuity, clinical outcomes (caries at recall), and patient-centeredness (self-management goal setting). Measure scores were highest for caries risk documentation (90-100%). For measures that were age-stratified, there were generally lower scores for adolescents than younger children. Quality improvement opportunities and measurement challenges were shared by the pilot sites.
Conclusions: The PCDH pilot sites successfully implemented a PCDH child measure set and identified improvement opportunities for its future use. Future directions include the addition of standards and patient-reported measures to finalize the PCDH measure sets.
